Dear Freshmen, Sophomores, and Parents/Guardians,
The tests will take place in the Moyer Lobby, and take approximately 25 minutes. Students do NOT need to come through the CapScann machines, but should enter the gate between the Smith Gym and Moyer Theatre. Timeslots are first-come, first-served.
Any students, including fully-remote students who plan to come to campus for activities (campus ministry, fine arts, clubs, athletics etc.) must be tested. If a student is not planning to come to campus at all this spring, no test is required.
Sign up links for Feb 13 & 15: We have reserved 8-8:30 am for sophomores attending the Sophomore Retreat on Saturday and Monday (2/13 and 2/15). Those students are getting a unique link for the 8 am time on the day of their retreat. For everyone else, please choose one of the following:
If they cannot make either of these dates, students must send documentation of a negative COVID test taken between Friday, February 13 and Monday, February 15 to reportcovid@jesuitportland.org

There is no charge for rapid antigen testing conducted by NW Mobile Labs. Jesuit is paying for the test administration. The pre-registration will ask for the student's insurance information. Your insurer will be reimbursed for the test itself through the federal CARES Act.
Students must complete a series of electronic forms before your COVID-19 test, at this link: https://bit.ly/nwmobiletesting_jesuit. The pre-registration will ask for the student's Social Security Number. If you do not want to give that, just enter 111 11 1111.
The demographic questions (ethnicity, gender, etc) are required by the State of Oregon, but students may skip those unless they choose to answer them. If students develop any symptoms between the time you fill out these forms and your test date, do not come to campus and let your doctor know.
Who should get tested? All students are required to receive a COVID-19 test prior to students' return to school. The only exception:

Day of Testing If you are exhibiting any symptoms (cough, fever/chills, muscle aches, headache, sore throat, diarrhea, nausea/vomiting, changes in smell/taste, runny nose/congestion, fatigue, chest pain/pressure, difficulty breathing/shortness of breath) or have recently been exposed to someone who has tested positive for COVID-19, please let your doctor know and do not come to campus for testing. You are required to wear a mask while on campus and practice social distancing.
Where to enter? Please arrive no more than 5 minutes prior to your reserved time slot as we will observe social distancing and safety protocols. Please enter via the gates between the Smith and PAC, and then enter the PAC through the doors right opposite the box office/Smith Gym.
After you enter the PAC, you will check in for your rapid COVID-19 test in the lobby and proceed to a testing station. After your test, you will exit through the back doors of the PAC and leave campus. Please do not stay on campus and mingle. This is very important for maintaining the health and safety of our community. Test results will be delivered to you via a secure e-mail within 24 hours of receiving your test. This email will be sent from nwmobiletesting@protonmail.com and you will use the following password to access the results: JesuitCovidResult In the event you receive a positive test result, NW Mobile Testing will call you on your provided contact number. You will be requested to receive a molecular PCR test through your health care provider. In the meantime, you will be asked to follow CDC recommended isolation procedures. If you feel sick or develop symptoms of COVID-19, you should contact your health care provider.
