Fall Workout Sign Up and Agreement
Important Notes:
SIGN UP IS DUE BY WEDNESDAY, SEPTEMBER 9th.
You may sign up for as many sports as you want - make sure you don't sign up for workouts that are at the same time.
You are committing to attend for all five weeks. (With the exception of Golf and Women's Tennis which have two and three dates, respectively.)
Note that for several sports, you will be contacted later with the exact location and/or time. This is because we need to balance numbers at each location and can't accurately do that until we know how many athletes have signed up.
The athlete AND parent must provide a digital signature for the Athlete & Parent Agreement.
